Anyone can stop the work when they see a problem
Give anyone an explicit stop signal when the AI, or the way it is being used, looks unsafe or wrong. The stop has to be honored immediately.

Try this first
Agree one signal that means stop, I have a concern. A word, a symbol, a raised hand. Then honor it every time.
Use it when
Confident-wrong output can ship fast and quietly, and the cost of a silent error is high.
Skip it when
If the last person who raised a concern got punished or ignored, fix that first. Until then the signal is empty and people will not touch it.
How to introduce it
Tell the team plainly. Anyone, at any level, can stop the work when something looks wrong with what the model produced or how someone is using it. Stopping is never punished. Raising the concern is the job.
How to show up
Set the rule and step back. The first time someone uses it, your reaction teaches everyone watching. Thank them in front of others, whether or not the concern held up.
How long it takes
About ten minutes to set up. It then stays in place indefinitely.
What makes it hard
The signal usually goes unused because people do not believe it is safe, not because nothing is wrong. Watch for a manager overriding a stop near a deadline. One override tells everyone the signal is decoration.
What it looks like when it's working
People use it, including juniors, and a stop produces a real second look instead of a fast reassurance.
How long until it sticks
This is a matter of culture, not skill. It becomes real the first time a stop is honored under genuine pressure. If a stop is ever punished or ignored, trust in the signal collapses and you begin again from the start.

Trust builds each time a stop gets honored. The strongest signal is a leader honoring one that costs them money.

A stop is valuable, not an interruption. One unnecessary stop costs almost nothing against one missed problem.
